你叫什么名字? — asking someone's name
To ask someone's name, say 你叫什么名字? The answer follows the pattern 我叫 + name.
你叫什么名字？我叫王小明。 — What's your name? My name is Wang Xiaoming.
认识你很高兴! — nice to meet you
When you meet someone for the first time, say 认识你很高兴! (Nice to meet you!) 认识 means "to know" or "to meet" someone.

你是哪国人? — asking someone's nationality
To ask where someone is from, say 你是哪国人? Answer with 我是 + country + 人.
